Yan Oi Tong Love Elderly Card Scheme

1. Introduction
This pioneer scheme was introduced in 2011, aiming to collect donations from the general public and to support those aged 65 or above from the single-person or the underprivileged families in Tuen Mun, Yuen Long, Tin Shui Wai and Tai Po.

An automated system matches the donor and the elderly in need, beneficiaries are able to obtain free services from YOT including medial treatments, meals, social and leisure activities by presenting the card. Thus, elderly social life are enhanced, quality of life are improved and neighborhood are connected.

The cases of the Scheme are referred by the social workers from the social welfare agencies, hospitals, elderly centres and YOT services units in the district of Tuen Mun, Yuen Long, Tin Shui Wai or Tai Po. Under the Scheme, there are two tiers of cards – Gold Card and Silver Card. Every Gold Card Holder will receive HK$1,200 donation per month while the Silver Card Holder will receive HK$700 per month.

We hope that more elderly in Hong Kong will benefit from YOT diversified services with donations under the Scheme.

 

2. Service Target Group

Aged 65 or above requiring financial assistance or from the underprivileged families in Tuen Mun, Yuen Long, Tin Shui Wai, and Tai Po.

 

3. Eligibility

  1. Aged 65 or above
  2. Hong Kong Permanent Resident
  3. Living in Tuen Mun, Yuen Long, Tin Shui Wai or Tai Po
  4. Monthly Income Limits:
    not more than HK$5,000 for elderly living alone
    not more than HK$9,000 for elderly couples
  5. Assets Limits:
    not more than HK$47,000 for elderly living alone
    not more than HK$65,000 for elderly couples
  6. Priority will be given to the elderly living alone or the elderly couple living together
  7. Not eligible to beneficiaries of Comprehensive Social Security Assistance (CSSA) Scheme

 

4. Features of Scheme
Information Technology Applications:
The beneficiary with the Card will be entitled to free medical treatments services, meals, and social or recreational activities after donor matching in the computer system.

Highly Transparent Donation System:
Donors are able to reach the information online for the use of the services among the beneficiaries on a real-time basis. A summary report with the conditions of beneficiary will be generated every 6 months.

Respectful Willingness of the Elderly:
With the guide and opinions from the professionals like social workers and staffs, the elderly will enjoy more flexibility in selecting the most appropriate service for themselves.

Comprehensive Care Management Approach:
SWD’s Comprehensive Care through the Case Management Approach was introduced, and the care of the needs from the elderly was emphasized. The six principles for the comprehensive care service – accommodation, living, support, happiness, contribution and education for the elderly are well considered.

Intervention of Social Workers:
An individual interview for the applicant and his/her family members will be arranged by a professional social worker, aiming to get more personal details and the needs of the applicant before case opened. All the information collected will efficiently facilitate the assessment and approval procedures. The Care Plan for the services required will be devised in the most appropriate manner.

Continuous Care Management System:
The social workers will understand the service needs of the card holders through the information in system.  This real-time information source is the best and crucial to the care of the elderly.
